Form W-4 (PDF) - To be completed by employee to identify federal income tax withholding.

Form W-9 (PDF) - To be used when hiring independent contractors or consultants. Persons who complete this form will receive a 1099-Misc statement from your company

Form I-9 (PDF) - Employment Eligibility Verification form. This form is mandatory for all newly hired employees. It is used to declare citizenship and eligibility to work in the United States.
